benefits are available to the employee and his or
her eligible dependents. This plan is designed to
reimburse the employee and covered family members
for certain medical and dental expenses. The current
health care plan booklet provides a schedule of
benefits and limitations. Certain coverage under
this plan may require employees to contribute to all
or part of its cost. |

Voluntary
Accident Insurance: This plan is
designed to pay a benefit in the event of the
employee's accidental death or dismemberment. The
premium for this coverage is paid by the employee.

Life Insurance:
A basic amount of life insurance is provided by CED.

Long-Term
Disability: This plan is designed to
provide a partial continuation of an employee's
income in the event he or she becomes totally
disabled. The waiting period for entry into this
plan is 25 months of continuous full-time service.

Pension Plan: Enrollment
is automatic after one full year of continuous
service. The entire cost of the plan is paid by CED.
An employee is vested in this plan after five years
of continuous service. The current plan booklet

401K Plan:
Enrollment is optional. An employee can participate
in the plan after one full year of service.
Employees can defer 1% - 15% of their annual W-2
earnings on a pre-tax basis. |
